Male patient, weighting 142.0 lb, was diagnosed with depression , anxiety  and was treated with Vistaril. After drug was administered, patient experienced the following side effects: condition aggravated, depression , suicide attempt. Vistaril dosage: unknown. During the same period patient was treated with TRAZODONE, KLONOPIN, SEROQUEL, WELLBUTRIN, PROZAC. Patient was hospitalized. Patient recovered.

After drug was administered, patient experienced the following side effects: anxiety , arthralgia, back pain , burn infection, depression , dyspepsia, dyspnoea. Vistaril dosage: 30 MG, HS. During the same period patient was treated with PARAFONTE, RESTORIL, COUMADIN, PAXIL, MACROBID, CALCIUM, MAGNESIUM, GINSENG. Patient was hospitalized. Patient died on 06/05/2002.
