Fortaz side effect was reported by a Pharmacist from UNITED STATES on Oct 18, 1993. Female patient, 61 years of age, was diagnosed with urosepsis, hypertension, atrial fibrillation What is  atrial fibrillation and was treated with Fortaz. After drug was administered, patient experienced the following side effects: clostridium colitis, renal failure, renal function test abnormal. Fortaz dosage: 500MG TWICE PER DAY. During the same period patient was treated with GENTAMICIN, LASIX, PREDNISONE, VANCERIL, ATROVENT, CAPTOPRIL, VERAPAMIL, HEPARIN. Patient was hospitalized and became disabled. Patient recovered.

During the same period patient was treated with GENTAMICIN SULPHATE, VALACICLOVIR HYDROCHLORID, ALBUTEROL, FUROSEMIDE, PREDNISOLONE, ACETAMINOPHEN, HEPARIN CALCIUM. Patient was hospitalized. Patient recovered.
