Fortaz dosage: unknown. During the same period patient was treated with ZOFRAN, CYTARABINE, MITOXANTRONE, CIFLOXECTION, URATE OXIDASEECTION, GEMTUZUMAB OZOGAMICINECTION, ACETAMINOPHEN.
