After drug was administered, patient experienced the following side effects: completed suicide, loss of consciousness. Cyclobenzaprine dosage: unknown. During the same period patient was treated with ACETAMINOPHEN, HYDROCODONE BITARTRATE, COCAINE, HYDROMORPHONE. Patient died.
