After drug was administered, patient experienced the following side effects: completed suicide, electrocardiogram qrs complex prolonged, hepatotoxicity, hypotension. Cyclobenzaprine dosage: unknown. During the same period patient was treated with ACETAMINOPHEN AND HYDROCODONE BITARTRATE, PROMETHAZINE, METOPROLOL. Patient was hospitalized. Patient died.
