Female patient, 33 years of age, weighting 240.3 lb, was diagnosed with dental cleaning, thyroiditis, osteoporosis , blood cholesterol, depression  and was treated with Aquafresh. After drug was administered, patient experienced the following side effects: autoimmune disorder, cerebrovascular accident, contusion, convulsion, diabetes mellitus, grand mal convulsion, joint swelling, loss of consciousness. Aquafresh dosage: unknown. During the same period patient was treated with AQUAFRESH WHITENING, SYNTHROID, MIACALCIN, CYTOMEL, LEVOTHROID, TRICOR, PAXIL, TRAZODONE. Patient recovered.

Aquafresh dosage: unknown. During the same period patient was treated with SEROQUEL, EFFEXOR, CALCIUM, SYNTHROID, KLONOPIN, LORATADINE. Patient recovered.
